To make an initial appointment you need a referral from another doctor. The referral can be addressed to Dr Hughes, Dr Dark, or North West Neurology. Referrals can be delivered by fax, post, or by hand to our office.

For your first appointment

For your first appointment at North West Neurology please:

• Advise the office of any paperwork you are bringing with you    
• Arrive 10 minutes early to fill out any necessary paperwork, or have this form filled out beforehand
• Bring your referral from your specialist or GP
• Bring a list of your current medications
• Bring copies of all scan images relevant to your condition if not available online
• Bring IPTAAS forms or any other forms that the doctor is required to complete

For any follow-up appointments

For any follow-up appointments at North West Neurology please:

• Bring your list of current medications
• Bring old scans and images
• Bring any new scans and images if not available online

If you have any further questions or concerns about your condition after your consultation, please write them down and book a follow-up appointment.
